Step 1 - Provide details for each student including date of birth and email address.
Step 2 - Provide us with the referral forms issued by your previous instructor or dive shop.
Step 3 - The course paperwork you signed for your original dive shop only covers their liability. We require you to repeat the process for us, with our details embedded in the fine print. This must be signed BEFORE your arrival.
We need your personal details to find or register you on the PADI System, match your paperwork to your profile and process your certification. IF YOU booked the course on behalf of other people, WE NEED YOU to fill them in here as well.
Your referral forms contain valuable information, helping us to tailor your remaining sessions to best meet your needs and ensure you meet all the requirements for certification. An example is shown below.
