All scuba courses that include in-water training—whether in a pool or the ocean—require each student to complete the industry-standard Diver Medical Questionnaire. This form asks a series of health-related questions to determine whether a medical evaluation by a physician is necessary before you can be cleared as “fit to dive.”
To simplify this process, we have provided a secure, privacy-focused digital version of the form below. It’s easy to complete and will automatically email you a confirmation indicating whether a physician’s approval is needed. If required, the official PDF version of the questionnaire will be attached to the email for your convenience—ready to print and take to your scheduled doctor’s appointment.
NOTE:
A doctor’s clearance must explicitly state that you are “fit to dive” without conditions. An example of a conditional would be "only if the person stays shallower than 60 feet or only if they keep their heart rate under 120 bpm"
